The Various cash benefits provided under the Scheme are distributed through a network of 41 Branch Offices and 8 Pay Offices, in U.P. Region, The Branch Offices are also the appropriate office for submission of Declaration Forms along with Return of Declaration Forms (Form-3) by the employers in respect of their employees coverable under the Act. The Branch Office are also appropriate Offices for submission of six monthly return of contribution in Form-5 in quadruplicate by the employers covered under the Act in that area. The Branch office, function under the supervision and control of the Branch Manager. The Branch Manager is empowered to admit claim related to Sickness Benefit, Maternity Benefit , temporary disablement benefit, funeral expenses and confinement expenses. He is also empowered to provisionally admit death cases caused due to Employment Injury if such accident occurs at the work place arising out of and in the course of employment
